Sponsorship Form

To make a donation, secure a sponsorship or make a pledge to participate in the 2021 OIC Annual Gala (Thursday, October 28, 2021), please provide the following information:

Sponsorship Levels

Presenting Sponsor  -  $5,000

Level 1 premier advertising company/names signage at two events; social media linkages, press releases, radio/web mentions; on mic announcement and promotions at the event; 8 tickets for the Gala and 6 tickets to Blues/Jazz Night on July 29th

Program Sponsor  -  $2,500

Level 2 premier advertising company name/signage at the event; social media linkages, press releases, radio/web mentions; on mic announcement and promotions at the event; 8 tickets for the Gala.

Dessert Sponsor  - $1,500

Level 3 advertising placement to include company name/signage on each table; social media linkages, press releases; promotions at event; 8 tickets for the Gala

Table Sponsor - $1,000

Level 4 advertising placement to include company name/signage with performace; social media linkages, press releases; promotions at event; 8 tickets for the Gala

Support Sponsor - $500

Earmarked table; 4 tickets for the gala
